Abuse, Neglect, and Exploitation Reporting Summary

This public-facing summary explains how GNHH identifies, reports, and responds to abuse, neglect, and
exploitation involving clients. It is designed for clients, families, representatives, caregivers, and community
stakeholders.
Purpose of This Summary
GNHH is committed to protecting the safety, dignity, and rights of all clients. Federal and state laws require
home-care agencies to immediately report any suspicion or knowledge of:
- Abuse (physical, emotional, verbal, sexual)
- Neglect (failure to provide needed care or supervision)
- Exploitation (financial, material, or personal benefit obtained through coercion or misuse)
This summary provides easy-to-understand instructions on how clients or their representatives can report
concerns.
What Counts as Abuse, Neglect, or Exploitation?
A. Abuse
Includes any intentional act or threat that causes harm, pain, fear, or distress, such as:
- Hitting, pushing, or any physical harm
- Verbal threats, yelling, humiliation
- Unwanted or inappropriate sexual contact
B. Neglect
When a caregiver fails to provide needed care or services, including:
- Withholding food, water, medication, or assistance
- Leaving a client unattended or in unsafe conditions
- Ignoring calls for help
C. Exploitation
Using a client’s money, identity, property, or belongings without permission, including:
- Taking cash, checks, or valuables
- Coercing a client to sign documents
- Misusing credit cards or personal information
Your Right to Safety
All GNHH clients have the right to:
- Receive safe, respectful, and dignified care
- Be free from abuse, neglect, exploitation, retaliation, or harassment
- Report concerns without fear of punishment
- Have concerns reviewed promptly and thoroughly
How to Report Abuse, Neglect, or Exploitation
Anyone—clients, family members, representatives, or the public—can report concerns.

- Verbally
- In writing
- By email
- Through a representative or advocate
Reports may be made anonymously, although providing contact information helps with follow-up.
External Reporting Options
Clients may also report abuse or neglect directly to state or federal agencies.
A. Adult Protective Services (APS)
For suspected abuse, neglect, or exploitation of adults.
APS hotline varies by state. GNHH will provide the appropriate number upon request.
B. Long-Term Care Ombudsman
For clients receiving home-care services who need assistance resolving disputes or concerns.
C. Local Law Enforcement
Call 911 if someone is in immediate danger.
What Happens After a Report Is Made?
While internal investigation procedures are handled confidentially, clients can expect the following general process:
- The concern is received and reviewed promptly
- Steps are taken to ensure client safety
- Necessary reports are made to state agencies
- The situation is evaluated and addressed
- Corrective actions are taken if needed
GNHH strictly prohibits retaliation against anyone who reports a concern.
Your Privacy
GNHH treats all reports confidentially to the extent permitted by law.
Information may be shared only with:
- Regulatory agencies
- Law enforcement (if required)
- Personnel necessary to address the concern
When to Call 911
If a client is in immediate danger, or if a crime is actively occurring:
CALL 911 IMMEDIATELY.
